Disulfiram is a drug prescribed to help manage chronic alcoholism by making the body react badly to alcohol. It blocks the enzyme aldehyde dehydrogenase (specifically ALDH2), so that even a small amount of alcohol triggers immediate hangover-like symptoms. These can include facial flushing, throbbing in the head and neck, headache, trouble breathing, nausea, heavy vomiting, sweating, thirst, chest pain, palpitations, shortness of breath, hyperventilation, rapid heart rate, low blood pressure, fainting, severe unease, weakness, vertigo, blurred vision, and confusion. In severe cases, the reaction may lead to respiratory depression, cardiovascular collapse, abnormal heart rhythms, heart attack, acute congestive heart failure, unconsciousness, seizures, and death.

Normally, the body breaks down alcohol into acetaldehyde, which is then processed by ALDH2 into a harmless substance. Disulfiram (through its metabolites, especially diethylthiomethylcarbamate) stops this second step, causing acetaldehyde to build up to levels five to ten times higher than usual. This buildup creates the unpleasant effects. The drug’s action mimics a common genetic condition—ALDH2 deficiency—found in about 540 million people of East Asian ancestry due to the ALDH2*2 allele. Reduced ALDH2 activity also occurs in about 120 million people of non-East Asian descent, though less frequently.

Disulfiram is a second-line treatment for alcohol dependence, after acamprosate and naltrexone. It does not reduce cravings for alcohol, so compliance is often poor. Methods to improve adherence include subdermal implants that release the drug for up to 12 weeks, or supervised administration—for example, by a spouse or pharmacist. If alcohol has been consumed in the last 12 hours, disulfiram should not be taken. The drug has no tolerance effect; the longer it is taken, the stronger its effects. Because it is absorbed and eliminated slowly, the reaction can last up to two weeks after the last dose. Medical ethics require that patients be fully informed about this reaction.
